I've tried this on Windows 7 Home Premium 64 bit, but it should also work for other versions of Windows 7, and probably also Vista if you have any issues there.
Step-by-Step guide:
1) Download Microsoft Application Compatibility Toolkit from Microsoft Download Center
2) Install it
3) Start "Compatibility Administrator" as Administrator. To do that, locate the entry in the Start menu, right-click it and select "Run as Administrator" from the context menu. Answer any questions like "Do you want to allow this program to make changes to your system" with Yes, now and later on.
4) On the left side, you'll see a tree view with an entry "Custom Databases". Below that should be an entry named "New Database(1) [Untitled 1]*" Right-click it, choose rename and rename it to something sensible (e.g. "Medieval")
5) Make sure that now renamed entry is selected, and click the "Fix" button in the toolbar. A dialog should appear.
6) Enter the name of the application and the vendor. Then click the Browse button and locate the file "Medieval_TW.exe" in your Medieval install folder. Select it and hit "Next"
7) Under "Operating Systems mode", select "Microsoft Windows XP (SP 2)"
8) Test the application by clicking "Test run...". A dialog asking for command line parameters appears. Just hit OK
9) The game should run now. It might take a little, be patient. Try out both the strategic map and a battle. When you are satisfied that the program runs, quit the game.
HINT: Do not try to switch between applications while the game is running. It might crash in that case, especially if you do it while it's switching graphics modes.
Now we have to apply the compatibility patch permanently.
10) Select "Next" twice, then "Finish" Now, you should see an entry for Medieval on the right pane of the Compatibility Administrator
11) Click "Save" in the toolbar to save the database. Save it anywhere you like, and with any name, but I would suggest the Medieval folder
12) Now, right-click the database you renamed in step 4 (the one with a yellow drawer on it), and select "Install" from the context menu.
